Delaware has historically had one of the highest infant mortality rates in the nation. That’s why community leaders saw a need for a program such as Nurse-Family Partnership. This compelled Children & Families First, a statewide nonprofit agency dedicated to helping families develop solutions to meet challenges and embrace opportunities, to launch Nurse-Family Partnership in 2010.

Over 40 years of evidence show the measurable, long-lasting difference Nurse-Family Partnership makes in the lives of moms and their children. Meet NFP moms and nurses in our video.

Sophonie and S’vanna
Sophonie, at her high school graduation, with her daughter, S’vanna

On June 1, 2018, Sophonie Celestin graduated from high school in Seaford, Delaware. She walked across the stage, wearing her cap and gown, as her  daughter, S’vanna, watched. Sophonie is a success: she accomplished her goals to finish high school on time and be the best mom she could be to her now 2-year old daughter. Sophonie faced opposition and challenges along the way to being a teen parent, but she is now proud to have her own place and looking forward to studying to becoming a nurse. She is doing it!
