In this role you will:

NFP provides home visits to first-time, low-income pregnant mothers and their families beginning in the first two trimesters of pregnancy and continuing until their child turns two years old. Nurse Home Visitors (NHVs) carry a caseload of up to 25 families and conduct 12-13 one-hour-long visits per week. NHVs also participate in community outreach and collaboration activities in their service area. These activities include meetings, presentations, phone calls, and emails.

During visits, NHVs use the nursing process to support the family in obtaining goals for themselves and their children. Typical assessments and screenings include blood pressure, infant growth, and feeding, parent-child interactions, intimate partner violence, substance use, mental health using PHQ-9 & GAD-& tools, infant development with Ages & Stages Questionnaires, and sensory screenings using OAE & SPOT machines. In response to assessments, nurses utilize interventions such as education, therapeutic communication, Motivational Interviewing, and community referral.

New NFP nurses receive rigorous onboarding and continuous education to support them in developing the skills necessary for success in such a unique role. This includes an initial one-week training at the NFP National Service Office in Denver, CO. Nurses attend weekly Team Meetings, Case Conferences with mental health consultation, and Reflective Supervision.

Nurse-Family Partnership is a national, evidence-based home visitation model. Program goals include improving outcomes in the following areas:
